John Lunn Flutes agrees to send a flute on trial for 10 days under the following conditions:

1. Print this form, fill it out completely and sign it. No P.O. boxes please, we cannot ship to them.

2. For insurance purposes, fill out the credit card info. No charges will be placed on the card.

3. Mail this completed form to us at the address below with a check for $50.00 to cover insured shipping to you.

4. After the trial period has ended, return the flute by UPS or Federal Express to ensure tracking and prompt delivery.
